
Southend-on-Sea Adult Social Care is committed to improving the way we support people. As part of this work, we have created a new strategy and delivery model called Connected Southend.
Connected Southend is the foundation for how we provide social work, occupational therapy, care services, commissioning, and support functions. It also links with our wider work across culture and arts in the city.
The aim of Connected Southend is simple:
to provide the best possible care, support, and advice for people in our city.
It is based on the first line of the Care Act statutory guidance:
“The core purpose of adult care and support is to help people to achieve the outcomes that matter to them in their life.”
We developed Connected Southend together with people who use our services, local community groups, and partner organisations. Their experiences and ideas helped shape every part of the model. Connected Southend also guides how our partners work with us, especially around integration and good practice.
The model sets out clear standards, principles, and ways of working for our staff. It places a strong focus on prevention and enablement, helping people stay independent and live the life they choose.
Connected Southend is our new approach to providing information, advice, prevention, support, and social care. It also reflects the heart of our city, through our support for voluntary and community groups, and our commitment to arts, culture, leisure, and heritage.
Our vision is to work alongside people so they can live safely, stay well, and remain independent in their communities. We want people to stay connected to what matters most to them and to live the life that is important to them.
