Continuity of care means you keep getting the care and support you need when you move from one council area to another in England.
There should be no break in your care when you move.
If you are moving to a different area

What will happen next
Southend-on-Sea City Council will share your information with your new council, including:
- your care and support plan
- your carer’s support plan (if your carer is moving with you)
- any other important information about your care
Before you move, your new council will:
- speak with you about your needs
- review your care and support
They may also visit you in your new home to check if your needs have changed.
