Our Adult Social Care Portal is the quickest and easiest way to make a referral for yourself or for someone you care for.
You can use the Portal to:
- request help or support
- make a referral for a care and support assessment
The Portal is a secure online system designed to make managing your social care needs simple and efficient.
Please note: The Portal is not monitored outside of business hours.

If you need urgent support when our offices are closed, please contact our Out of Hours Service:
