Progressions (Polaris Childrens Services)
Progressions is a service that provides support to 16 to18 year olds (or up to 25 with an additional needs diagnosis) to transition back into the community with more independence. Referrals mostly come from CAMHS where young people do not fit the criteria for adult mental health. We do offer a self-referral, where young people or other professionals can complete a referal. In order to access our service the young person who is being referred needs to have had a previous intervention/assessment with CAMHS.
As a service our aim is to empower young people with their transition into adulthood. The support Progressions offers is both emotional and practical with main goals be working towards support around education, employment, building on self-esteem/confidence, emotion regulation or practical support to build on their independence.
