Harley Weaver, a Social Worker in the Southend-on-Sea City Council Children with Disabilities Team provided information about the work of the team. The presentation covers:

Presentation 2: Transition from the Children with Disabilities Team to Adult Social Care
Members of the Southend-on-Sea City Council Adult Social Care Team spoke about the process of young people transitioning from the Children with Disabilities Team to the Adult Social Care Team, and support available from the team. The presentation covers:
- how the team use conduct Care Act Assessments and create transition plans
- what is covered in the assessment and how it is tailored to support the individual

Health: Support for children and young people's mental health and wellbeing
This session covered some of the services available in Southend to support children and young people's mental health and wellbeing.
Presentation 1: Overview of support for children and young people's mental health and wellbeing in Essex
Sue Holland, a Commissioning Manager at Essex Integrated Care Board (ICB), opened the session.
In this presentation, Sue gave an overview of the demographics of the local area and the key priorities of Essex ICB. She talked about the Start Well and Feel Well elements of the ICBs work, giving an overview of the areas of work within these strands, and introduced the I Thrive model.

Presentation 2: Support from the Southend School Nursing Service for children and young people
The Team Lead for the Southend School Nursing Service provided information about support available from the team. The presentation covers:
- the schools supported by the service
- numbers of children and young people in the local area
- the emotional and wellbeing offer for children and young people
- referral routes and how to access the service
- Chat Health text messaging service for children and young people and parent carers

Presentation 3: Support for children and young people from the Essex Mental Health Support Team
Marie Henderson, Locality Clinical Lead for the South East Essex Mental Health Support Teams provided an overview of the service. The presentation covers:

- what the service can help with at primary and secondary school age

Presentation 4: Overview of the Mid and South Essex Children and Young People Counselling Service
A representative of Kids Inspire provided information about the Mid and South Essex Children and Young People Counselling Service. The presentation contains information about the partner organisations who provide the service, who the service is for, what the service provides and how it is accessed.

Presentation 5: Support for children, young people and families from SNAP Charity
Kate Cachia from SNAP, a special educational needs and disabilities support charity based in Brentwood, provided an overview of support available for families through the charity. She also talked about the facilities available at the centre and how many families they support.

Education: Reasonable adjustments to support students with SEND - what parent/carers can expect
This talk covered topics including what is a reasonable adjustment and the types of adjustments that can be made.

Clair Warner, Head of the Specialist Teaching Service at Southend-on-Sea City Council spoke about the adjustments that schools can reasonably be expected to put in place for pupils with special educational needs and/or disabilities. The presentation covers:

- the school environment
- what is reasonable to expect in a mainstream school

Bethan Mort, Head of the SEND Therapies Team at Southend-on-Sea City Council spoke about how the team support schools to make the environment more accessible and inclusive for pupils. The presentation covers:

- supportive equipment that can help pupils with SEND
- sensory spaces and tools
- time out arrangements and spaces
