- Description
-
The Southend, Essex and Thurrock (SET) DSR (Dynamic Support Register) Keyworker Service provides support to children and young people (CYP) who have a learning disability or are autistic and are:
- inpatients in a mental health hospital
- at risk of being admitted to a mental health hospital
- placed on the Dynamic Support Register (DSR)
What we do
The primary goal of the service is to reduce the number of CYP in an inpatient setting, reduce unnecessary or avoidable admissions and support step-down transition for CYP into the community as early as suitable.
Who we are
We are a small team of keyworkers and senior keyworkers who will aim to ensure children, young people and their families voices are heard and that they have access to the right support at the right time.
We can act as the main point of contact for children and young people and their families to ensure a joined up approach to support.
A Keyworker will work alongside children/young people and their families to identify personal goals and put in place their own unique action plan. They will help breakdown any barriers and will support children and young people to achieve their goals.
