Key dates and transition planning for young people with Education, Health and Care Plans

For young people with Education, Health and Care Plans (EHCPs), it is especially important to plan ahead to ensure a smooth transition to post-16 education.

Here are some key dates and actions to keep in mind:

Autumn Term:

  • attend school sixth form/college open events and explore options (view information about open events here)
  • EHCP transition annual reviews should take place this term
  • share chosen pathways with your EHCP co-ordinator at the local authority

By mid to late December:

  • aim to view and apply for preferred courses/settings
  • this ensures time for consultation and planning

By February (at the latest):

  • submit applications to chosen settings
  • share application details with the Preparing for Adulthood (PFA) team at the local authority

By 31 March:

  • the young person's chosen post-16 setting should be named in their EHCP

Early applications for courses and settings are essential to allow time for the local authority to consult with the setting and confirm the placement in line with statutory deadlines.
