Some children and young people (up to age 18) have very complex health needs.

These may be the result of:

  • conditions present at birth
  • long-term or life-limiting conditions
  • disability
  • serious illness or injury

These people may need extra health support, beyond that which is ordinarily available from:

  • GPs
  • hospitals
  • in the community

This extra package of care is called continuing care.

Who is eligible for continuing care?
