Developing the action plan
Following the identification of key priorities through meaningful engagement with young people and their families, a draft action plan was co-produced with input from a broad range of stakeholders, including education, health, social care, and voluntary sector representatives. This collaborative approach ensured that the proposed actions were not only aspirational but also practical and achievable within existing resources.
To refine the plan, targeted consultation sessions were held with stakeholder groups to test feasibility, identify potential barriers, and explore opportunities for joint delivery. Feedback from these sessions informed the final version of the action plan, which includes:
- clear objectives aligned to each priority area
- named leads responsible for delivery
- timeframes for implementation
- success measures to evaluate impact
This structured approach ensures that the action plan remains focused, accountable, and responsive to the lived experiences of those it aims to support.
Accountability and monitoring
The SEND Strategic Partnership Board will oversee the implementation of the strategy and action plan, ensuring that progress is tracked and reported transparently. Monitoring will take place at least termly and will include:
- progress reports from lead agencies against agreed milestones
- data analysis to assess outcomes and identify areas for improvement
- feedback loops from young people, families, and practitioners to inform ongoing refinement
- challenge and support from Board members to maintain momentum and address barriers
To promote transparency and shared ownership, a summary of progress will be published annually and shared with all stakeholders, including young people and families. This will include case studies, performance data, and reflections on what has worked well and what needs to change.
The Board will also ensure that the strategy remains dynamic, with the flexibility to adapt actions in response to emerging needs, policy changes, or feedback from the community.
