Description

The Southend, Essex and Thurrock (SET) Keyworker Service provides support to children and young people who have a learning disability or are autistic and are:

  • inpatients in a mental health hospital
  • at risk of being admitted to a mental health hospital
  • placed on the Dynamic Support Register (DSR)

The service aims to ensure children, young people and their families:

  • have their voice heard
  • access the right support at the right time
  • have a main point of contact
  • identify personal goals and create an action plan

A Keyworker will work alongside children/young people and their families to identify personal goals and put in place their own unique action plan. They will help breakdown any barriers and will support children and young people to achieve their goals.

Referral notes

Referrals into the service arrive via the Essex Dynamic Support register and other professional services.

Whilst we are not able to accept direct referrals into the Keyworker Service, self-referrals and professional referrals can be made to the Essex Dynamic Support Register. For details please contact the DSR team Essex.DSR@essex.gov.uk
